To enter the sweepstakes, click on where indicated or print your name and address including city, state and zip code) on a 3" x 5" card and mail it to: BULLET FOR MY VALENTINE CONTEST, c/o Alternative Press Magazine, 1305 W. 80TH ST. STE. 2F CLEVELAND, OHIO 44102. Enter as often as you like, but each entry must be handwritten and mailed separately. Mechanically reproduced entries will not be accepted. Entries by mail must be postmarked by 3/20/08. Entries will not be returned. No responsibility is assumed for lost, mutilated, late, or misdirected entries. 2. Prizes. 1 Grand Prize(s) will be awarded - 3 RUNNERS UP. Winners will be determined at a drawing on 3/20/08. All prizes will be awarded. 3. Second Chance Drawing. All prizes not claimed by 4/3/08 will be awarded in a second chance drawing on 4/4/08. Winners in the second chance drawing will be notified by mail by 4/5/08. Participants in the second chance drawing will consist of all non-winning entrants. They will be automatically entered in the second chance drawing. Winners in the second chance drawing must claim their prizes by 4/12/08. Any prizes not claimed by that date will not be awarded. 4. Odds of Winning. The odds of winning will be determined by the number of entries. 5. Eligibility. Sweepstakes is not open to employees (or members of their immediate families) of RED MUSIC, BULLET FOR MY VALENTINE, AP. Winners must sign an affidavit of eligibility and a publicity release allowing the use of their names by the sponsor. Void where prohibited by law. All taxes are the responsibility of the prize winners. 6. Winners' List. For a list of winners, send a stamped, self-addressed envelope to CONTEST WINNERS LISTS, c/o Alternative Press Magazine, 1305 W. 80TH ST. STE. 2F CLEVELAND, OHIO 44102 by 9/20/08 (six months after the drawing date), OR CHECK WWW.ALTPRESS.COM FOR COMPLETE LIST OF WINNERS. Allow 3 weeks for delivery of the winner's list. 7. Sponsors.
